HASTINGS SCHOOL DISTRICT #200 INTENT TO DESTROY 1992 RECORDS

In accordance with applicable state and federal regulations, the Hastings Public Schools does hereby notify the public of its intent to destroy student records from: Graduation Year 1992.

The records to be destroyed include, but are not limited to, documentation contained within the child's / parent's temporary student record.

A copy of the student transcript will, however, be maintained by the district permanently following the student's graduation, transfer or withdrawal from the District.

Minnesota Law requires that the district provide prior written notice of its intent to destroy student records and that each student or resident be given an opportunity to obtain copies of all records to be destroyed.

How to get a copy of the records to be destroyed:

Please submit your request by emailing Cathy Blaha at cblaha@hastings.k12.mn.us or in writing to:

ISD #200 - Attn: Cathy Blaha,

1000 West 11th Street,

Hastings, MN 55033

within the next 30 days. Please include the following information in your request:

•Name

•Name of the student as it would have appeared on the file (maiden name if applicable)

•Your current mailing address

•Daytime phone number where you can be reached.
