YOU are hereby summoned and required to answer the Complaint in the above-entitled action, which Complaint has been filed in the Office of the Court Administrator of said Court, and to file your Answer to said Complaint in the Office of said Court Administrator within twenty (20) days after service of this Summons upon you, exclusive of the date of service.

If you fail to answer said Complaint within the time aforesaid, the Plaintiff above-named shall apply to the Court for the relief demanded in the Complaint and shall move to have default judgment entered against you.
